Flu Shot Information

The Smith County Health Department and the Smith County School System have partnered to provide seasonal influenza (flu) vaccinations at your child’s school. Information is attached. If you would like for your child to receive the flu vaccine, please completely fill out the the attached consent form. The consent link will close on 9/25. Be sure to sign the form as this will be your permission for your student to receive the vaccine.
