FINAL FORMS IS NOW OPEN FOR THE 26-27 SCHOOL YEAR

Final Forms allow you to complete and sign athletic participation forms for your student-athletes. For returning athletes this should be a very quick process. For incoming 9th graders and new student-athletes you must complete these forms before you will be allowed to try out. ALL STUDENT-ATHLETES MUST PRINT THE TSSAA PPE PHYSICAL FORMS(FOR PHYSICIANS). This can be printed on Final Forms once all documents have been completed.
